With a problem stated, a change course sends you to the evidence, and it grades the trip in three parts. The search: where you looked, with what terms, filtered how, so that a reader could rerun it. The appraisal: what each retrieved study actually is, its design, its setting, its size, its measures, and what that design can and cannot support. The synthesis: what the pile of studies, weighed rather than counted, tells your specific setting to try. Written work in this territory usually takes the form of an appraisal matrix, an evidence summary, or a short review that ends in a practice recommendation.

The distinction the week turns on is between evidence that supports a practice and papers that mention it. Change questions in nursing pull up a mixed catch: a few controlled trials, many single-site improvement reports, some observational work, reviews of varying discipline, and editorials wearing reference lists. The graded move is sorting the catch honestly. An improvement report from one 30-bed unit is real evidence with a real boundary, and stating the boundary is what appraisal means. A student who presents five such reports as five independent proofs has counted; a student who notes they share a design weakness and weighs them accordingly has appraised.

Fit is the third axis, and the one most drafts skip. Evidence transfers on mechanism, not on brand: a practice that worked through daily physician engagement will not transfer to a setting that cannot supply it.   2. Convert your problem into a searchable question

    Population, intervention territory, outcome: the week one problem statement supplies all three. Write the question before the search terms, because terms drift toward what is findable and the question is what keeps them honest.

  3. Run the search and write down what you did

    Databases, terms, limits, years, and the counts at each narrowing step. Reproducibility is the standard, and the search paragraph is the cheapest full-credit material in the whole deliverable.

  4. Label every retrieved study by what it is

    Trial, cohort, before-and-after report, review, guideline. One label per source, applied before reading for findings, because the label sets how much the finding can weigh. Most appraisal errors are labeling errors made silently.

  5. Appraise with a named tool, honestly

    Use the appraisal instrument your course provides or names, answer its questions as asked, and let a weak study be weak. An appraisal in which every study scores well is reporting the student's hopes, and graders in this territory read it that way.

  6. Synthesize by weight and fit, then recommend

Evidence craft when evidence is the subject

Read past the abstract before a study enters your matrix. Abstracts round results upward and bury exclusions. The methods section is where a study tells you what it actually did, and appraisal graded at masters level assumes you met the methods, not the summary.

Reviews are evidence about evidence, so cite them at their level. A systematic review supports statements about the state of the literature. It does not become a trial when convenient, and citing a review's included study as though you appraised it yourself is the kind of shortcut this week is designed to surface.

Guidelines are consensus documents with dates and issuers. They belong in the base as the expectation-setters they are, cited with body and year, and their recommendations trace to underlying studies you can follow when a claim matters enough.

Absence of evidence is a findable, citable result. If the search returns little, say so with the search shown, and treat thinness as a finding that shapes the project, smaller claims, closer measurement, rather than as a failure to be hidden with padding.

Five mistakes that cost points in this week's territory

  • Collecting instead of appraising. Ten sources summarized without designs named is a bibliography with commentary. Five sources weighed with boundaries stated is the assignment.
  • Vote counting. Six studies for and two against is not synthesis when the six share one weak design and the two do not. Weight is the whole point of appraisal.
  • Grade inflation across the matrix. When every study is strong, the appraisal has stopped discriminating, and the grader knows the literature better than that.
  • Ignoring fit. A practice proven in a setting your unit does not resemble, recommended without the mechanism argument, leaves the recommendation standing on borrowed ground.
  • A recommendation from nowhere. The final paragraph must be traceable to the weights you assigned. A recommendation that ignores your own appraisal, usually toward the popular practice, forfeits the synthesis row it sits in.

How many sources is enough for the evidence base?
Your week's guide sets any minimum, and the honest answer above the minimum is: enough to show the shape of the literature, appraised deeply, which usually means five to eight key sources handled properly rather than fifteen handled thinly. Depth is what the rows pay. If your search legitimately returns more strong work than you can appraise, say so and select on relevance and design, stating the selection rule, which is itself appraisal behavior a grader can credit.
What do I do when the studies contradict each other?
Treat the contradiction as data about the practice rather than as an obstacle to your paper. Look first at design: findings from stronger designs outrank weaker ones, and much apparent contradiction is a trial disagreeing with hopeful before-and-after reports, which is not a tie. Look next at setting and dose: the same practice run with different intensity, staffing or duration is not the same practice, and divergent results often map to divergent implementations. Write the disagreement out in exactly those terms, then let your recommendation carry the uncertainty honestly, perhaps by proposing closer measurement in your own setting. A well-analyzed contradiction is one of the highest-scoring things this week can contain.
Can I include improvement project reports, or only research studies?
Include them, labeled as what they are, because for many change questions they are most of what exists. An improvement report tells you a practice was implemented somewhere real, with real constraints, and what the site observed afterwards, which is genuinely useful for feasibility and mechanism. What it cannot do is separate the practice from everything else that happened at that site, and your appraisal should say so in one plain sentence. The failure mode is not inclusion, it is promotion: an improvement report cited with the verbs of a trial, demonstrated, proved, reduced, has been silently upgraded, and this course grades exactly that verb choice.
