What NR-539 Week 1 asks for
Consider a family practice group carrying about 1,400 children under twelve across three offices. The chart review shows 214 of them with a persistent asthma diagnosis, and 78 of those 214 filled a controller inhaler fewer than four times in the preceding twelve months. That is not yet a planning problem. It becomes one when you say which of those 78 you intend to reach, over what period, to move which number, and why the reason they are not refilling is something a program can touch. Everything graded in this course descends from that single act of narrowing, and a first-week paper that skips it leaves seven later stages resting on air.
The graduate move in an opening planning stage is from description to specification. Description says childhood asthma control is poor in the practice. Specification says that among the 78 children with fewer than four controller fills, the practice has no recall process after a missed follow-up visit, that 41 of the 78 are in one office serving a ZIP code where the nearest pharmacy closes at six, and that the design target is a refill rate rather than an education score. A grader reading the second version can already see where a plan would attach. A grader reading the first version can only see a topic.
Deliverables at this stage usually run modest: a short problem statement, sometimes a revised population profile carried forward from the assessment sequence, often a posted response introducing the population you intend to work on for the rest of the session. Treat the post as final copy. Posts do not reopen after submission in Canvas, and in a planning course an early public commitment to a vague population is expensive, because your classmates and your faculty will hold you to the wording for eight weeks.

Reduce each rubric row to its verb before drafting anything
Copy the scoring rows into a blank file in their printed order and strip each to the action it demands: define, justify, prioritize, support. A row that says prioritize is asking you to reject alternatives on stated grounds, not to explain why your topic matters.

Fix the population boundary with a numerator and a denominator
Write one sentence naming who is inside the group, who is outside it, over what time window, and how many people that leaves. If you cannot state the denominator, you cannot later state a rate, and every measurable target in this course is a rate.

State the gap as two numbers, not one adjective
Give the observed value and the comparison value in the same sentence: your practice, your county, a state figure, a published benchmark. A problem is the distance between them, and the distance is what your plan is later scored against.

Sort the causes by the level they operate on
Separate what sits with the family, what sits with the clinic process, and what sits with the surrounding structure of transport, hours, cost and coverage. This sorting decides which interventions are even eligible, and it prevents the education-for-a-structural-barrier mismatch that this course penalizes hardest.

Choose one determinant and say out loud what you are setting aside
Name the one you will build on, then give two sentences on the ones you are not addressing and why: outside your scope, outside the session's window, already owned by another program. Explicit exclusion reads as judgment; silent exclusion reads as an oversight.

Evidence craft for a planning problem statement
Every rate arrives with its base, its source and its date. Seventy-eight of 214 children over twelve months from the practice's own pharmacy fill feed is evidence a reader can weigh. Thirty-six percent is a number that has been cut loose from everything that made it meaningful. In a planning course the base is not decoration, because the denominator you state in week one is the denominator your evaluation has to use in week six.
Name the surveillance source rather than the summary that quoted it. Population figures move through several hands before they reach a news story or an advocacy page. Cite the agency dataset, the state health department report or the peer-reviewed analysis, give the collection year in your sentence, and say plainly when the newest available figure is three years old. A stale number honestly dated is a defensible foundation; a fresh-sounding number with no provenance is not.
Keep local observation as illustration, never as the rate. What you have seen in a pediatric session is legitimate material for one sentence that makes the problem concrete. It cannot substitute for the measured value, and a paragraph that runs on clinical impression alone will lose the support row no matter how accurate the impression is.
Match the comparison to the population. A national figure compared against a three-office practice panel is a weak contrast, because the two groups differ in age mix, coverage and geography before any program exists. Where you can, compare against the same measure at county level or against a benchmark published for a similar setting, and say in one clause why the comparison is fair.
Five mistakes that cost points in this week's territory
- A population that cannot be counted. Children in underserved communities has no denominator, so nothing you propose later can be measured against it.
- Importance in place of a gap. Paragraphs establishing that childhood asthma is a serious condition answer a question the scoring rows did not ask.
- Determinants left unsorted. A single list mixing parental knowledge, clinic recall processes and pharmacy hours hides the level mismatch that decides the whole plan.
- A target with no measure attached. Improve asthma control cannot be evaluated, and week six will have nothing to build a data collection plan around.
- Carrying forward an assessment that was never tightened. If the earlier course left the population loose, repair it in one paragraph now rather than building seven stages on it.
