NR-503 Week 2 Incidence, Prevalence and Rates: How to Write It

The short answer

NR-503 Week 2 is the measurement stage, and it is the one that decides how the rest of the session reads. Incidence counts what arose during a period and answers questions about risk. Prevalence counts what exists at a moment and answers questions about burden. Around them sit the ratios, proportions and rates, the choice of a denominator, mortality and case fatality, and the reason a rate reported per 100,000 needs both a population and a set of years attached. What NR-503 Week 2 asks for

The stage is technical and the deductions are clean, because a misused measure is not a matter of interpretation. Expect to distinguish a ratio from a proportion from a rate, to compute or interpret incidence and prevalence, and to explain the relationship between them: a condition with low incidence can carry high prevalence because people live with it for years, and improved survival raises prevalence without anything getting worse.

The denominator is the recurring test. Incidence needs a population at risk, which excludes anyone who already has the condition and anyone who cannot get it. Prevalence needs the whole population at the point of counting. Mortality needs the population, while case fatality needs only the people who had the disease. Getting the bottom of the fraction right is most of the work, and a paper that reports a percentage without saying what it was a percentage of has already lost the row.

Deliverables at this point in an eight-week session often mix computation with interpretation, sometimes as a worksheet with a written explanation, sometimes as a short paper reporting measures for your population. If your section runs a discussion this week it will usually ask for a statistic. Verify the figure against its source before you paste, because posts do not reopen once submitted in Canvas and a wrong denominator sits there for the session.

The NR-503 Week 2 method, step by step

Six moves that keep the arithmetic and the sentences agreeing with each other.

  1. Decide what the sentence is trying to prove

    Risk, growth or the effect of something recent needs incidence. Burden, service demand or capacity planning needs prevalence. Choose the measure from the claim rather than from what the source happened to publish, and where the source gives only one, adjust the claim rather than stretching the measure.

  2. Write the denominator before the numerator

    Say who is in the bottom of the fraction and why, then count the top. For incidence that means removing existing cases and anyone not at risk. Doing it in this order catches most of the errors that would otherwise appear three paragraphs later as a conclusion that does not follow.

  3. Fix the time window explicitly

    Incidence is meaningless without a period. Prevalence is meaningless without a point or a defined interval. Write the window into the sentence rather than into a footnote, and keep it consistent when you compare two figures.

  4. Show the calculation if you make one

    Numerator, denominator, multiplier, result. A bare answer cannot be scored for reasoning and a shown one can earn partial credit even if the arithmetic slips. Say which multiplier you used, since per 1,000 and per 100,000 change the appearance of a figure entirely.

  5. Interpret in a separate sentence from the number

    State the figure. Then state what it means for the population. Then stop, before the sentence starts claiming a cause. Blending the number and its meaning into one clause is how measurement papers acquire conclusions their data never supported.

  6. Compare like with like

    Before setting two figures beside each other, check that the age structures, the years and the case definitions match. A crude rate in an older community compared against a national age-adjusted rate is a comparison that proves nothing, and saying so is worth more than the comparison would have been.

Evidence craft for reported measures

Report the case definition alongside the figure. Two sources counting the same condition can disagree because one counted diagnosed cases and the other counted people meeting criteria on a survey instrument. Naming the definition in a clause is what lets a reader see whether two numbers were ever comparable.

Say whether a rate is crude or adjusted. Age adjustment exists because populations differ in structure, and comparing an unadjusted local figure against an adjusted national one is a common way to manufacture a difference that is not there. One word in the sentence prevents it.

Give the reporting cycle when you give the year. Surveillance data is published on a schedule, so the most recent complete year is often two or three years behind now. State the years the figure covers, then say what may have changed since and in which direction, and a limitation becomes an appraisal.

Keep the verb inside what the design supports. Counting exercises describe. They do not establish that anything caused anything. Was higher among and increased between these years are honest; caused and drove are not, and this is the stage where that habit either forms or does not.

Five mistakes that cost points in this week's territory

  • Incidence and prevalence used as synonyms. They answer different questions, and the swap usually invalidates whatever conclusion was built on it.
  • Existing cases left in an incidence denominator. The population at risk excludes people who already have the condition, and forgetting that inflates the figure in a way a grader can check.
  • A percentage with no base. The most frequent deduction in this course, and the easiest to prevent by writing the count first and the percentage second.
  • Multipliers swapped mid-paper. Reporting one figure per 1,000 and the next per 100,000 without saying so makes two comparable numbers look a hundred times apart.
  • Rising prevalence read as a worsening problem. Better survival raises prevalence. Without incidence beside it, the claim that a condition is spreading has no support.

  • Each measure is named correctly and answers the question the sentence around it asks
  • Every denominator is stated, and the incidence denominator excludes existing cases
  • Every figure carries a time window and a multiplier
  • Any calculation shows numerator, denominator and multiplier rather than only a result
  • Comparisons are checked for matching years, age structures and case definitions
  • No sentence claims a cause on the strength of a count

Questions students ask about this stage

My source only reports prevalence. Can I still write about risk?
You can write about risk, but not from that figure alone, and the honest move is to say so in the paper. Prevalence tells you how many people currently have the condition, which is a statement about burden, not about how likely someone is to develop it. Two routes are open. The first is to look for an incidence figure from a different source and to note openly that the two figures come from different collections with different definitions and periods. The second is to keep the prevalence figure and rewrite the claim as a burden claim: this many people in this group are living with the condition, which is what drives service demand. Both score. What loses points is leaving the prevalence figure in place under a sentence about rising risk, since the number cannot support the word above it.
How much arithmetic will I actually have to do?
Less computation than most students fear and more interpretation than they expect. The graded skill is choosing the right measure, attaching the right denominator and period, recognizing when a comparison is fair, and saying what a figure does and does not show. Where a calculation is required it is usually a single division and a multiplier rather than anything statistical. Show your working when you do one, because a shown calculation can earn partial credit if the arithmetic slips and a bare number cannot. Keep a running note of every figure you use with its source, its years, its denominator and its multiplier, since the same figures tend to reappear in later stages of the session and rebuilding that list from scratch is the most avoidable time cost in the course.
What multiplier should I use when reporting a rate?
Use the one your source used, and say which it is. Changing the multiplier is legitimate arithmetic but it invites error and it makes your figure hard to check against the original. The convention is to pick a multiplier that leaves the number readable, which means per 1,000 for common conditions and per 100,000 for rarer ones, and public sources generally follow that. Where you must convert to compare two figures, show the conversion rather than presenting the converted number alone. And keep one multiplier for the whole paper wherever possible, because a reader comparing your figures will not stop to notice that the scale changed between two paragraphs, and a grader who does notice reads it as carelessness rather than as a deliberate choice.
