What NR-439 Week 6 asks for

A policy review binder lands on a nurse's desk with two flagged attachments. One is a recent systematic review supporting the practice under review; the other is an older single-site trial that found no benefit. The committee chair has written one line on the routing slip: which do we believe? The wrong answer is a coin flip between the two. The right answer treats them as unequal witnesses, one summarizing many studies systematically, one reporting a single small experiment from years earlier, and weighs them accordingly. Learning to make that weighting explicit, on paper, with reasons, is what this week grades.

The deliverable at this stage is usually a synthesis paper or an evidence table with accompanying prose, built from the articles your search produced: each source ranked by its level on the evidence hierarchy, its key finding distilled to a line, and then, in the writing, the findings compared across studies rather than summarized one at a time. Some sections also ask for an explicit statement about the overall strength of the body of evidence. Whatever the exact assignment, the structural demand is the same: this week's unit of analysis is the finding, not the article, and papers organized as one-paragraph-per-study are the week's most predictable disappointment.

The hierarchy itself is quickly learned and easily misused. Systematic reviews and meta-analyses of trials sit at the top because they aggregate many experiments; individual randomized trials sit below them; then quasi-experimental work, then observational studies, then descriptive and qualitative work, with expert opinion at the base. The level answers exactly one question: how well protected is this design against the errors that produce false conclusions about interventions? It does not say whether a given study was done well, whether its population resembles yours, or whether its question matches your question, and the strongest submissions this week use the hierarchy as one input among several rather than as a verdict machine.

Expect disagreement in your evidence set and treat it as material rather than misfortune. Two studies that point in different directions force the synthesis question the week exists to teach: why? Different populations, different doses or intensities of the intervention, different outcome definitions, different settings, different years. Locating the likely source of a conflict, and saying which finding deserves more weight and why, is the highest-value paragraph available to you this week, and students who flatten the conflict into vague agreement leave those points on the table.

Six moves that turn a stack of summaries into one synthesis.

  1. Assign every source its level before rereading any of them

    Work through the set once, labeling each study's design and hierarchy level with a one-line justification. Disagreements between what an article claims to be and what its methods describe get resolved in favor of the methods, exactly as in the reading weeks.

  2. Build the evidence table as your working document

    One row per study: citation, level, design, sample, key finding in a single sentence, and a fit note for your question. The table is not decoration for the appendix; it is the instrument that makes cross-study patterns visible enough to write about.

  3. Extract the findings and group them across studies

    List every distinct finding your set contains, then note which studies support each one. Those groupings, three studies on this outcome, two on that barrier, one dissenting, are the outline of your synthesis, generated mechanically from the table.

  4. Write each paragraph around a finding, citing several studies

    The synthesis paragraph opens with the claim, then marshals its supporters with their levels and, where it exists, its dissenter. If most of your paragraphs cite only one source, the paper has slid back into serial summary, and rubrics for synthesis catch exactly that.

  5. Work the conflicts instead of smoothing them

    For each disagreement, name the most plausible explanation from the studies' own differences, population, intensity, measurement, setting, and state which side carries more weight in your judgment and why. A reasoned adjudication is worth more than any amount of manufactured consensus.

  6. Grade the body of evidence honestly at the end

    Close with a strength statement: how many studies, at what levels, how consistent, how close to your population. Strong, moderate, or limited, with the reasons shown, and with the gaps named as gaps rather than hidden.

Evidence craft for writing above single studies

Carry each study's level into the prose. When a finding rests on a systematic review, say so in the sentence; when it rests on one descriptive study, say that too. The reader of a synthesis should never have to leave the paragraph to know how much weight a claim carries.

Count your support honestly. Three studies is three studies, not a growing body of research. Report the actual number behind each synthesized claim, because inflating quantity with vague plurals is the synthesis-week version of an unsourced assertion, and graders with your table in hand can count.

Keep qualitative findings in the synthesis without promoting them. If your set includes interview or experience studies, synthesize them where they belong: explaining mechanisms, barriers, and patient perspectives alongside the outcome evidence. Let each type carry its proper load, and say explicitly which type is carrying which claim.

Cite every study each time it supports a claim. In finding-organized writing, one study may appear in four paragraphs, and each appearance needs its citation. The reference list stays the same length; the in-text citations multiply, and that multiplication is what synthesis looks like on the page.

Five mistakes that cost points in this week's territory

  • Serial book reports. One paragraph per article, in search order, is the structure this week exists to replace, and no strength of individual summaries redeems it.
  • The hierarchy as a gag order. Discarding every source below the top levels, instead of weighting them, throws away the mechanism and context evidence your recommendation will need.
  • Conflicts smoothed into mush. Writing that studies show mixed results, without explaining or adjudicating the mix, stops exactly where the graded thinking starts.
  • A table that contradicts the prose. Levels or findings that differ between your evidence table and your paragraphs tell the grader one of them was not checked.
  • Strength inflated at the close. Declaring strong evidence over two small studies burns the credibility the whole paper was building, one sentence before the end.

Questions students ask about this stage

Is a higher-level study always the one to believe?
No, and the exceptions are exactly where graders look for judgment. The hierarchy ranks designs by their structural protection against error in answering intervention questions, but a level is not a quality certificate, and it is not a relevance certificate either. A systematic review built on a handful of weak, dissimilar trials can support less than its level suggests; a well-conducted cohort study in a population nearly identical to yours can inform your question better than a trial run in a setting nothing like your own; and for questions about experience or mechanism, the designs at the bottom of the intervention hierarchy are the right instruments entirely. The disciplined move is to weight by level first, then adjust openly for quality and fit, writing the adjustment down: this trial outranks the cohort study by design, but its population differs from ours in ways that matter, so both inform the conclusion. That visible reasoning is the graded commodity.
What do I do with one strong study against three weak ones?
Resist the arithmetic instinct, because evidence is weighed, not tallied. Three descriptive studies agreeing do not outvote one well-run randomized trial on an intervention question; the trial's design protections are the point of the hierarchy, and your synthesis should say plainly that the strongest evidence points where the trial points. But do not discard the three, because agreement among weaker studies still carries information, especially about feasibility and generalizability across settings, and their consistency with or against the trial is itself a finding worth a sentence. The paragraph that scores describes the full geometry: what the strongest source shows, what the weaker sources add or contest, and what would settle the remaining doubt. If the weak studies all disagree with the strong one, that tension deserves your best explanatory paragraph rather than a silent omission.
What if my whole evidence base turns out to be inconclusive?
Report it as the finding it is, and understand that the assignment grades your synthesis skill, not your luck in question selection. An honest conclusion that the evidence is limited, a few small studies, mixed directions, populations unlike your own, demonstrates every competency the week teaches: leveling, comparison, adjudication, and honest grading. Close the paper with what inconclusiveness implies for practice, because it implies something real: current practice continues with awareness rather than confidence, any local change would be experimental and should be treated as such, and the specific gaps you name are what future research or a local quality project would need to address. What you must not do is inflate the verdict to make the paper feel more satisfying, because the distance between your evidence table and an overclaimed conclusion is exactly one rubric row wide, and graders measure it.
