The seventh stage of a systems leadership course usually asks for the implementation layer: how an interprofessional change is actually put into a working setting, how it spreads beyond the site that piloted it, and what makes it survive the departure of the person who championed it. Implementation science gives this stage its vocabulary, and the practice-doctorate framing matters here more than anywhere in the session. You are translating existing evidence into a local practice change and evaluating it, not running a trial. Your section may print this as NR 721 or NR721; it is the same course. What NR-721 Week 7 asks for
Why do changes that worked disappear? A mobile clinic team serving three shelters introduced a standardized pre-visit huddle in which the outreach worker, the nurse and the driver spent four minutes agreeing on who they expected to see and what each person needed. It was popular, it cut duplicate paperwork, and the numbers improved. Fourteen months later it was gone. Nothing dramatic happened. The nurse who ran it moved to a day shift, the huddle depended entirely on her arriving ten minutes early, no document described it, no orientation mentioned it, and no metric reported it. The change had been implemented and never institutionalized, which is a distinction with an entire literature behind it and the distinction this stage exists to teach.
The territory here is threefold. Implementation covers how a change gets into the work: the strategies used, the sequence, the roles assigned, the supports supplied. Spread covers what happens when a change moves from the unit that grew it to units that did not, where local adaptation and fidelity pull against each other. Sustainability covers what makes it persist: documentation, orientation, measurement, budget and ownership that outlive individuals.
Deliverables at this stage tend to be an implementation plan or a plan for spread and sustainability, often built on the design you proposed earlier in the session and often expected to use a named implementation or improvement framework. Frameworks are not decoration here. A plan that names its framework and then organizes itself by that framework's stages or domains is measurably easier to score, and doctoral rubrics generally reward the discipline directly.
The practice-doctorate boundary deserves stating plainly because students blur it constantly at this stage. A quality improvement or practice-change evaluation is not a research trial. You are not testing a hypothesis about the world, you are asking whether a known-good practice took hold here, with what fidelity and with what effect on local measures. That means no promises about generalizable findings, no language borrowed from experimental design that your plan cannot support, and no assumptions about how any oversight body would classify your work. Where a project might require review, the correct sentence is that the determination belongs to the appropriate review process, not that approval will be obtained.
One more thing to build in early: the plan should be written so that a reader can see who does what on the first day, in the first month and in the sixth. Plans that describe intentions without an actor and a date are the most common shape of a weak submission at this stage.

Name the framework and organize the plan by it
Choose one implementation or improvement framework, cite it with its year, and use its domains or stages as your structure. A framework mentioned in the introduction and abandoned by page two is worse than none.

Separate the intervention from the implementation strategies
The intervention is what changes for patients. The strategies are what you do to make the change happen: training, audit and feedback, champions, reminders, workflow redesign. Confusing them is the most common conceptual error in these papers.

Assign an actor and a date to every action
Role rather than name, and a week rather than eventually. A plan that reads staff will be educated has not said who educates, in what format, or when they are released from clinical duty to attend.

Define fidelity and decide what may be adapted
State the core elements that must be preserved for the change to work and the peripheral ones a second site may alter. Spread fails when everything is negotiable and when nothing is, and saying which is which is a doctoral-level judgment.

Build the measurement plan before the go-live
Process measures for whether the change is happening, outcome measures for whether it is working, and balancing measures for what it might be breaking elsewhere. Name the data source and the interval for each.

Write sustainability as ownership, documentation and budget
Who owns the change once the project ends, which standing document describes it, where it appears in orientation, and which recurring report carries its measure. If none of those exist, the change ends when you do.

Evidence craft for implementation writing
Match every strategy to a barrier you already evidenced. The strongest structural device available in this paper is a sentence of the form: because the readiness assessment found that no one owns the record template, this plan assigns that ownership before go-live. It ties the session together and it makes the strategy defensible rather than generic.
Use improvement and implementation vocabulary precisely. A plan-do-study-act cycle, a pilot, a phased rollout and a trial are different things with different claims attached. Doctoral graders notice when experimental language is used for a quality improvement project, and it is one of the more damaging errors available.
Give every measure a denominator and a source. The proportion of eligible visits with the new step completed, drawn from the visit report, reviewed monthly. That sentence contains everything a reader needs. A measure named without its source cannot be audited and will be marked as incomplete.
Write the review question without promising its answer. Where human subjects review or a quality improvement determination might apply, say that the project will be submitted for determination through the appropriate process, and describe how participant or staff information would be protected. Never write that approval will be granted or that review will not be required.
Five mistakes that cost points in this week's territory
- Education as the whole strategy. Teaching people about a change is one strategy among many and the one least likely to hold on its own.
- A framework named and then ignored. If the plan's headings do not follow the framework, the citation is decorative.
- Research language for a practice change. Hypotheses, arms, control groups and generalizable findings do not belong in a translation project and signal that the doctoral distinction has not landed.
- No balancing measure. Every change costs something somewhere, and a measurement plan that cannot detect harm looks naive to an experienced reader.
- Sustainability written as hope. Continued leadership support is not a mechanism. Ownership, documentation, orientation, budget and a recurring report are.
