NR-670A now asks how anyone would know whether the change worked, and the answer has to be written before any data exists. A measurement plan states what is counted, how it is defined so two people would count it the same way, how often, by whom, and what result would count as improvement. Written after the fact it is a rationalization; written in advance it is a design. Your section may print this as NR 670A or NR670A; it is the same course. What an evaluation design has to protect you from
On an inpatient behavioral health unit, a nurse leader auditing safety round documentation gave the same twenty records to a colleague and compared results. They agreed on fourteen and disagreed on six, every disagreement turning on the same question: whether a round documented outside the expected interval counted as completed or not. Nobody had ever written the rule down. One paragraph defining the measure, including the interval and what to do with entries falling outside it, eliminated the disagreement entirely. That paragraph is the intellectual centre of this stage, and it is worth more than any statistical technique a practicum student could apply.
The purpose of a measurement plan is to protect a conclusion from being unfalsifiable. If the definition is loose, the result can be read whichever way the writer prefers, and a grader who notices will discount everything that follows. If the definition is tight, the result stands whether it is favorable or not, and a capstone that reports an unfavorable result against a tight definition is a better document than one reporting improvement against a definition that shifted. Concluding leadership experiences are assessing exactly that judgment: whether you can design an evaluation that could have contradicted you.
Three kinds of measure belong in a serious plan and students usually write only one. Outcome measures tell you whether the thing you care about moved, and in a short practicum they usually cannot. Process measures tell you whether the change you designed actually happened, and they move first, which is why they carry most of the weight in an eight-week window. Balancing measures tell you whether the improvement created a cost somewhere else, such as time added at a handoff or a step displaced onto another role. Naming a balancing measure is one of the clearest markers of graduate-level thinking available in this stage, and its absence is one of the most common deductions.
The practicum boundary holds. Your 144 hours, the hour log, activity counts, site documentation and the mentor evaluation are your own record of work you personally performed, never drafted, reconstructed or estimated with help from anyone. Any data collection you carry out is real practicum activity conducted under your site's permissions and your program's expectations, and nobody else collects it, supplies figures, or reconstructs numbers you did not gather. What the written layer supports is the design and the reporting: how to write an operational definition, how to choose measures that can move inside your window, and how to state decision rules and limitations honestly. All data reaching your paper is de-identified and reported in aggregate, with no case described specifically enough to be recognized.

Go back to the unit you defined in the problem statement and keep it
The thing you counted to establish the gap is the thing you count to detect the change. Redefining it between stages is the most common reason a capstone cannot demonstrate its own result, and it is invisible to the writer until the closing paper refuses to add up.

Write an operational definition a second reviewer could apply cold
Numerator, denominator, what is included, what is excluded, where the data lives, over what interval, and who collects it. If your colleague could take the definition and produce your number without asking you a question, it is finished. If not, the ambiguity will surface later as a disagreement about results.

Add a process measure tied directly to the behavior you designed
Your implementation plan named something a role does differently at a specific moment. The process measure is the proportion of eligible episodes in which that behavior occurred. In a short practicum this is the measure most likely to show anything, and it is the one that tells you whether an absent outcome means the change failed or never happened.

Name at least one balancing measure and mean it
Ask what your change costs somebody: minutes added at a step, a task pushed to another shift, a delay introduced at a handoff. Then define it as precisely as the others. A balancing measure that is genuinely capable of embarrassing your plan is the one worth having.

Decide the display before the first data point exists
Say whether you will show a run chart across intervals, a simple before and after comparison with both denominators, or a small table. Choosing in advance stops the presentation from being selected later to flatter the result, and describing the choice in the plan is itself a scoring point.

Write the decision rules and the limitations in advance
What result would count as improvement, what would trigger a redesign, and what you would conclude if nothing moved. Then list the threats you already know: small numbers, a short window, a single site, and the fact that you collected the data on your own project. Naming those in advance is credibility; discovering them in the closing paper is damage control.

Evidence craft for evaluation writing
Define first, count second, always in that order. A number produced before its definition was written is a number whose meaning was settled after the fact. Writing the definition into the paper, in full, is what allows a reader to accept the count without having watched you collect it.
Small denominators are honest, hidden denominators are not. In a short practicum the bases are small, and that is expected. Report 7 of 31 rather than 23 percent, say what the window was, and let the reader see the scale they are being asked to reason about.
Match your causal language to your design. A before and after comparison on one unit cannot support caused or produced. It supports was associated with, followed, and coincided with. Overreaching on a single verb undoes an otherwise disciplined evaluation, and it is the deduction graders make most readily at this stage.
Declare the collector, especially when it is you. A process measure audited by the person whose project it is carries a known bias, and one sentence naming it demonstrates the data literacy the row is looking for. Where a second reviewer checked a subset, say how many and how often you agreed.
Write the de-identification and storage sentence explicitly. State that records are reviewed without identifiers, that aggregate counts alone appear in the paper, and that nothing is retained beyond what your site permits. That sentence protects you, and it also demonstrates that you understand data handling as a leadership responsibility rather than an afterthought.
Five mistakes that cost points in this week's territory
- Outcome measures only. Nothing moves fast enough in eight weeks, and without a process measure a flat result cannot be interpreted at all.
- A definition two people would apply differently. If reviewers can disagree, the number is not a measurement, it is an opinion with a decimal point.
- No balancing measure anywhere. A plan that cannot conceive of its own cost reads as advocacy rather than evaluation.
- A shifted denominator. Counting all episodes at baseline and only eligible ones afterwards produces an improvement that exists entirely in the arithmetic.
- Success criteria written after the results. Deciding what counts as improvement once you know what happened removes the possibility of the evaluation ever having failed.
