NR-670 Week 4 Evidence Synthesis: How to Write It

The short answer

Around the midpoint of NR-670 the written work turns outward. The task is a synthesis: gathering the published evidence about the change you intend to make, judging its quality, and writing an argument that says what the literature collectively supports and how confidently. A synthesis is not a set of article summaries with headings. It is a claim built from sources that sometimes disagree. Your section may print this as NR 670 or NR670; it is the same course. What a synthesis has to prove

A director of case management once described the moment her capstone changed shape. She had eleven articles in a folder and a document with eleven subheadings, and her mentor read two pages of it before asking a single question: which of these disagree with each other. Nothing in the folder answered that, because nothing in the document had been compared. Rebuilding it around three claims instead of eleven authors took an afternoon and turned an annotated list into an argument.

Synthesis at this stage carries a specific burden of proof. Your reader wants to know whether the intervention you are about to propose has support, how strong that support is, in what populations and settings it was demonstrated, and what conditions the successful implementations had in common. The last of those is what separates a leadership synthesis from a research one: you are not only asking whether an intervention works, you are asking what has to be true of an organization for it to work there.

Expect the deliverable to be a written review with citations, frequently accompanied by a table that arrays the studies against common columns, and sometimes a short posted response naming the strongest source you found. Length rises again here. Reference-list discipline matters more than at any earlier stage, because a synthesis with a citation in the text that never appears in the list, or a list entry that appears nowhere in the text, loses a mechanical row that costs the same points as a reasoning error and takes ten minutes to prevent.

The practicum boundary continues to hold. Hours, activity logs, site documentation and any evaluation your mentor completes are your own verified record and are never drafted, reconstructed or estimated with help. This stage is almost entirely a written and library task, which makes it the safest place in the course to work with a tutor on structure, argument and citation mechanics. Where your synthesis touches your own setting, keep the local detail de-identified and aggregate, exactly as in the earlier stages.

The NR-670 Week 4 method, step by step

Six moves that convert a folder of articles into a defensible argument.

  1. Write the searchable question from your problem statement

    Population, intervention, comparison and outcome, taken directly from the gap you defined earlier. A question written after the searching is done is the reason so many syntheses drift away from the project they are supposed to support.

  2. Record the search so it could be repeated

    Databases, terms, limits, date range, and how many results were screened down to how many included. Capstone rubrics frequently score search transparency, and it is the one part of this stage that costs nothing but discipline.

  3. Build the matrix before you write any prose

    One row per source, columns for design, sample, setting, intervention detail, outcome measured, effect found, and quality judgment. Filling it forces you to read the methods rather than the abstract, and the patterns become visible without effort.

  4. Sort by finding, never by author

    Read down each column and group the sources by what they show. Three studies that agree become one paragraph with three citations. That single reorganization is the difference between a summary and a synthesis, and graders can see it from the shape of the page.

  5. Weight the evidence out loud

    Say which sources carry the most weight and why: design, sample size, setting similarity, recency. A leadership synthesis that treats a multi-site trial and a single-unit improvement report as equivalent has not done the appraisal the course expects.

  6. End with the implementation conditions

    Across the successful studies, what was in place: leadership sponsorship, an electronic prompt, dedicated time, audit and feedback. That list is the bridge to your week five plan and is the most useful paragraph in the whole synthesis.

Evidence craft for synthesis writing

Group citations behind claims, not claims behind citations. A sentence that ends with three sources is a synthesis sentence. A paragraph that opens with an author's name and walks through their study is a summary paragraph, and a page of them will be scored as an annotated bibliography no matter how good the individual readings are.

Report effects in clinical units before statistics. Say that the intervention reduced discrepancies by roughly a third, or shortened the interval by eleven minutes, then supply the statistical detail. Leadership readers act on magnitude, and a synthesis carried entirely by p values does not tell anyone whether the change is worth doing.

Name disagreement and explain it from the methods. Where two good sources conflict, the explanation is usually in the population, the dose or intensity of the intervention, the outcome measured or the follow-up window. Finding it is high-value analysis; declaring one study more trustworthy on instinct is not.

Keep improvement reports in their own weight class. Single-site quality improvement work is legitimate and often the most practically informative evidence you will find, and it cannot rule out concurrent change. Use it for feasibility and implementation detail, and say plainly which claims rest on it.

Check the reference list mechanically before submission. Every in-text citation appears in the list, every list entry appears in the text, every author-date pair matches, and every source is one you actually read rather than one quoted inside another paper. Secondary citation without attribution is the quiet integrity failure of capstone writing.

Five mistakes that cost points in this week's territory

  • One paragraph per article. The structure itself announces that no comparison happened, and no amount of good prose inside the paragraphs recovers the row.
  • A search that cannot be repeated. Reviewed current literature is not a method; databases, terms and screening counts are.
  • Silence about conflicting evidence. A synthesis where every source agrees usually means the disagreeing ones were never opened.
  • Ignoring setting mismatch. Evidence from a tertiary academic center applied to a rural clinic without comment is a transferability failure a grader will name.
  • No bridge to implementation. A synthesis that stops at the intervention works leaves week five to start from nothing.

Questions students ask about this stage

There is almost no literature on my specific problem. What now?
Widen one dimension at a time and say in the paper which dimension you widened. If nothing exists on your exact process in your exact setting, search the same process in adjacent settings, or the same failure mechanism under a different name. Medication reconciliation at post-acute transfer, for example, sits inside a much larger body of work on transitions of care, handoff communication and information transfer at discharge, and the mechanisms transfer even when the setting does not. Write the widening explicitly: the direct evidence is limited to a small number of reports, so the synthesis draws on the broader transitions literature for mechanism and on the direct reports for feasibility. That is a stronger paper than one which pretends to more direct evidence than exists, and the sentence naming the limitation usually earns the analysis row rather than costing it.
How many sources does a capstone synthesis need?
Your rubric may set a floor and that floor is the answer for scoring purposes, but the useful question is different: enough to support each claim you make, with the strongest available evidence for the central one. A synthesis with eight well-chosen sources organized into four claims reads better than one with twenty-two sources arranged as a list, because the second usually contains several that were never really read. Prioritize recency where practice has changed, prioritize design strength for the effectiveness claim, and keep one or two implementation-focused reports for the feasibility argument. If you are padding the count, the honest fix is another claim rather than another citation on an existing one, and the matrix will tell you quickly which claims are thin.
Can I include my organization's internal data in the synthesis?
Include it, and keep it in its own category rather than mixed into the published evidence. Internal audits, dashboards and quality reports establish your local problem and belong in the sections about your setting and applicability; they are not literature and should never be cited as though they were. The clean structure is to keep the synthesis about published evidence, then use a clearly labeled paragraph to compare that evidence against your own measured baseline. Attribute internal figures as internal, state the period they cover, keep the organization unnamed if your program requires it, and make sure nothing in the description could identify a patient or a colleague. That separation also makes week five easier, because the plan you write then has to bridge exactly the two bodies of evidence you have just kept distinct.
