The short answer

Executives receive problems as complaints and can only act on them as numbers, and the conversion between the two is the most heavily weighted piece of writing in the first half of a nurse executive concluding experience. A baseline says how often something happens, to whom, over what period, out of how many opportunities, and what the current state costs in hours, dollars, capacity or risk. Without that paragraph a proposal has nothing to measure improvement against and no argument that the problem is worth anyone's money. What NR-631 Week 2 asks for

Take a telehealth nurse line that everybody agrees is overwhelmed. As a complaint it is unusable. As a baseline it becomes something an executive can act on: of 3,140 calls offered in a quarter, 412 abandoned before answer, abandonment concentrated between 16:00 and 19:00 on weekdays, with average speed to answer in that window running roughly three times the daytime figure. Nothing in that sentence is an opinion. It identifies where the problem lives, how large it is, and where an intervention would have to be aimed, and it establishes the exact measure any later evaluation will report against.

The written work at this stage asks for four things in sequence. A definition of the measure precise enough that two people would count the same way. A source for the data with its window stated. A distribution rather than an average alone, because averages conceal the pattern that makes a problem tractable. And a cost of the current state expressed in units the organization recognizes: labor hours, agency spend, avoidable bed days, penalty exposure, or capacity that cannot be sold.

The last of those is the one students skip, and it is the row that quietly decides the grade. A proposal that cannot price the present has nothing to weigh its solution against. Costing does not require a finance qualification; it requires arithmetic done transparently and assumptions stated openly. If 412 abandoned calls in a quarter mean some proportion of those patients presented somewhere else, saying what proportion you assume and why is honest analysis, and a reader can adjust your figure using their own assumption if they disagree.

Expect a written problem statement or baseline section as the deliverable, sometimes with a table or a simple chart, and possibly a posted response describing the problem. Keep any post consistent with the numbers in your paper, since posts do not reopen after submission in Canvas and a figure quoted casually will still be visible when your final proposal reports a different one.

Everything drawn from your organization must be de-identified. Describe the setting by type and scale rather than by name, refer to roles by function, and ensure no patient, staff member or incident could be identified from any figure or example you include.

The NR-631 Week 2 method, in six analytic moves

Six moves that turn a complaint into an operating number.

  1. Define the measure until two counters would agree

    Write the definition as an instruction: which events count, which are excluded, from what start point to what end point. Ambiguity here is invisible in week two and fatal in week six, when the same measure has to be used for target setting.

  2. Name the data source and its window explicitly

    Which report, which system, which period, and who owns it. A number with no traceable source cannot survive a question in a committee, and the proposal you are writing is a rehearsal for exactly that question.

  3. Report the distribution before any average

    Break the total down by shift, day, unit, service or category and show where the problem concentrates. Concentration is what makes a problem solvable, and an average that hides it produces an intervention aimed at nothing in particular.

  4. Convert volume into a cost with visible arithmetic

    Show the multiplication. If a delay consumes an estimated fraction of an hour of nursing time and occurs a stated number of times per quarter, put the numbers and the rate in the sentence so a reader can recalculate with their own assumptions.

  5. Label every assumption as an assumption

    Where you estimate rather than measure, say so in the same sentence and give the basis. Executives are comfortable with estimates and hostile to estimates presented as facts, and graders in this course read the same way.

  6. Establish the comparison the number should be judged against

    Benchmark, internal target, prior period or peer unit. A figure with nothing to compare it to cannot be called a problem, and the comparison you choose is itself an analytic decision worth a sentence of justification.

Evidence craft for baselines

Counts before proportions, always. Of 3,140 calls offered, 412 abandoned tells a reader everything. A 13 percent abandonment rate tells them one thing and hides the volume that determines whether the problem is worth a budget line.

Attribute benchmarks with their source and year. A comparison figure drawn from a professional body, a national data set or a published study needs its origin named inside the sentence, because a benchmark from an unstated source is an assertion rather than a standard.

State data limitations before someone else finds them. If the report undercounts because events are captured only when someone files them, say so and estimate the direction of the error. A baseline that acknowledges its own weakness is more persuasive, not less.

Use the organization's own vocabulary for units. Worked hours per patient day, length of stay, agency percentage, throughput, avoidable days. Writing in the units a finance office already reports makes your proposal legible to the people who would fund it.

Five mistakes that cost points in this week's territory

  • A problem statement made of adjectives. Frequent delays and significant frustration cannot be measured, targeted or reduced.
  • An average with no distribution. A mean wait across a whole week hides the two shifts where the problem actually lives, which is the information an intervention needs.
  • No cost of the current state. The row that asks what the problem costs the organization is the one most often left effectively blank, and it is heavily weighted.
  • Estimates dressed as measurements. A figure derived from an assumption and presented as data is the error that damages credibility fastest with an executive reader.
  • Benchmarks with no source. The national average is around is a sentence a committee will stop you on, and a grader will mark the same way.

Before you submit

  • The problem appears as a measurable statement in the first two sentences
  • The measure is defined precisely enough that two people would count identically
  • Every figure carries its denominator and its time window
  • The distribution is shown, not only the total or the average
  • The cost of the current state is calculated with visible arithmetic
  • Every assumption is labeled as one, with its basis
  • No patient, staff member or organization is identifiable in the text

Questions students ask about this stage

I cannot get access to the data I need. What now?
Work with what is obtainable and be explicit about the substitution. Three routes usually remain open. Publicly reported organizational measures exist for many quality and safety indicators and can establish that a problem is real even when internal detail is unavailable. A short structured observation or count conducted during your own immersion, described honestly as a limited sample over a stated period, is legitimate evidence at this scale. And published literature can establish the typical magnitude of a problem in comparable settings, which lets you say what is likely true here even without local confirmation. Whichever you use, name it plainly, state the window, and say what a stronger baseline would have required. A proposal that says the ideal measure was not accessible and therefore a two-week structured count was used is far more credible than one that quietly presents an estimate as a system figure.
How do I cost something that has no obvious dollar value?
Convert to time first, then decide whether to convert time to money. Almost every operational problem consumes labor: minutes spent chasing information, repeating handovers, waiting for a decision, or redoing work. Estimate the time per occurrence, multiply by the frequency you established in your baseline, and you have an annual hours figure that is meaningful on its own. Whether to attach a dollar rate depends on your setting and your audience; some organizations prefer hours, and using a published average wage figure with the source named is acceptable where money is expected. For risk-based problems, the honest treatment is exposure rather than expected cost: how many patients were affected, what the potential consequence is, and what a single serious event would mean. State that as exposure explicitly, since presenting a risk estimate as a savings figure is the error most likely to be challenged.
My problem is real but the numbers are small. Does that weaken the proposal?
Not if you frame the significance correctly. Small numbers with severe consequences are exactly the kind of problem executives fund, and the argument is about severity and exposure rather than volume. Four events in a year involving a high-harm process may justify more attention than four hundred minor inefficiencies, and saying so directly is stronger than inflating the count. What you must not do is stretch the definition of the measure to make the number look larger, because a measure that includes near misses, related-but-different events and informal reports will be picked apart the moment someone asks what exactly was counted. Keep the definition tight, report the number as it is, and make the case on the consequence, the trend if one exists, and the regulatory or professional standard that makes even a small number unacceptable.
