What NR-520 Week 7 asks for
A measurement plan has five parts, and almost every weak draft is missing the same one. Name the outcome in observable terms. Name the measure and where the data lives. State the baseline and the period it covers. Set the interval at which you will look again. Then say what size of change you would treat as real rather than as ordinary variation. That last item is the one nearly everybody omits, and it is the difference between a plan that could be evaluated and a promise to evaluate.
The measure family distinction carries the second half of the work. Outcome measures tell you whether the thing you care about changed. Process measures tell you whether the change actually happened, which matters because most improvements fail at implementation rather than at concept. Balancing measures tell you whether you broke something else in the attempt. A plan with only an outcome measure cannot tell a failed idea from an idea nobody performed.
Data sources deserve more care than students usually give them. A number that comes from a chart audit, a number that comes from an administrative extract and a number that comes from a survey have different meanings, different lags and different reliability. Saying where a figure will come from, who will collect it and how often is not administrative detail; it is the part of the plan that determines whether the plan is real.

Name the exact figure, its numerator, its denominator and the system or audit it comes from. A measure without a source is an aspiration, and a grader can see the gap immediately.

Establish a baseline with a stated window
Write that 27 events occurred across 4,100 patient days in the six months before the change. A baseline quoted as a bare percentage cannot be compared with anything, because the comparison depends on the base and the period.

Add a process and a balancing measure
One measure of whether the change was actually performed, and one measure of the harm you would most fear causing elsewhere. Three well chosen measures beat eight listed, and the balancing measure is the one that signals maturity.

Set the interval and the decision rule
How often you look, and what you would do at each look. Continue, adjust, stop. A plan that collects data without a rule for acting on it has built a report rather than an evaluation.

State the result that would tell you the change did not work. Writing that sentence is what converts a plan into an inquiry, and it is the move most likely to lift a paper out of the middle band in this course.

Evidence craft for quality science
Every rate carries a denominator and a window. Twenty seven events across 4,100 patient days over six months is a figure. Six percent is a rumor. Comparing two rates built on different bases is the most common analytic error in this territory and the easiest for a grader to mark.
Improvement designs cannot pay for causal verbs. Rates fell after implementation is honest. The intervention reduced the rate claims a counterfactual that a before and after design cannot supply. This is the course where that distinction is explicitly on the sheet.
Name the design of any improvement report you cite. Published quality work is frequently quoted as though it were trial evidence. Write that a single site interrupted time series tracked an outcome for 18 months before and after a change, then the result, and the reader can weigh it properly.
Distinguish variation from change. Two data points are not a trend. Where your plan will look monthly, say how many points you would need before you believe a shift, and say what ordinary month to month movement looked like in the baseline period.
Five mistakes that cost points in this week's territory
- An outcome that cannot be observed. Improved satisfaction, better teamwork and increased awareness are topics, not outcomes, and no measure can be attached to them as written.
- A baseline with no base. A percentage quoted without its count and its window cannot anchor a comparison, which makes the whole evaluation unfalsifiable.
- Outcome measures only. With no process measure, a null result cannot be told apart from a change nobody actually performed.
- No threshold for a real change. Without a stated rule, any movement in the desired direction becomes success, which is exactly the reasoning quality science exists to prevent.
- Causal verbs on a before and after design. The reader does not need to know your setting to mark the overclaim, because the design and the verb are both on the page.
