A workflow is a sequence of actions performed by named roles, each one producing or consuming information, and the analysis consists of writing that sequence accurately enough that a reader could redraw it. Underneath all of it sits the distinction between what a policy says happens and what actually happens on the unit at two in the afternoon.

The deliverable usually asks for a described or diagrammed process with an analysis attached, and where a figure is invited, build it after the prose rather than before, since drawing exposes the step your sentences skipped.   2. List every role that touches the information

    Include the ones nobody thinks of: unit clerks, transport, pharmacy technicians, environmental services, the person who answers the phone. Papers that reduce the cast to nurses and physicians miss most of the handoffs, and handoffs are where the marks are.

  3. Walk it in real time rather than from memory

    Follow one instance of the process with a notepad, or reconstruct it with somebody who does a different part of it than you do. Write what actually happened, including the workaround everybody uses and nobody documents.

  4. Mark every handoff and every wait

    Put a symbol at each point where information changes hands and each point where something sits waiting for a person or a system. Then time the waits, even roughly. The distribution of waiting almost always surprises the person who mapped it.

  5. Locate the break at a step, not in a system

    Name the single step where information stalls, duplicates or degrades. It has to be a step, because steps can be changed and systems cannot, and a paper that concludes the process is inefficient has not finished the analysis.

  6. Change one thing and name the measure

    Propose a single alteration at the step you identified, then state what number you would watch to know whether it worked and how long you would watch it. One change with a measure beats five recommendations with none.

Evidence and citation craft for observed processes

Say whether you watched or remembered. Observation and recall produce different maps, and a reader is entitled to know which one they are reading. One sentence stating that you followed three instances over two shifts, or that you reconstructed the sequence from experience, sets the weight of everything that follows.

Timings need a method behind them. If you report that a step takes twenty minutes, say how you know: timed instances, a system timestamp, or an estimate from the people doing it. Estimates are acceptable when labeled and worthless when presented as measurements.

Published process work carries its setting with it. Before importing a finding about a workflow from the literature, state the unit type, the staffing arrangement and the shift pattern it came from. Processes are shaped by staffing more than by software, so a result from a different staffing model may not transfer at all.

A policy describes intent, not practice. Citing the procedure document tells the reader what should occur. Your map tells them what does. Use both, keep them clearly separate, and treat the gap between them as material rather than as an embarrassment.

A diagram earns nothing until the text uses it. Where you include a figure, refer to it by number at the point in the analysis where the reader needs it, and make sure every element in the drawing appears in the prose. A picture attached to the end of a paper is decoration.

Five mistakes that cost points in this week's territory

  • A process with no boundaries. Medication administration is a category, not a process. From order entry to first dose on this unit is something a reader can hold you to.
  • The map drawn from the policy manual. A sequence copied out of a procedure document describes an ideal nobody follows exactly, and it produces an analysis of a process that does not exist.
  • A cast of two. Nurses and physicians only. The clerk who reroutes the fax, the technician who reprints the label and the aide who notices the missing armband are usually standing at the break point.
  • The failure blamed on the system as a whole. The system is slow is not a finding. The finding is that a result posts to a queue nobody watches between shifts, which is a sentence somebody could act on tomorrow.
  • A redesign that only adds. Recommending an extra verification step, an extra field and an extra huddle without removing anything makes the process longer and the compliance worse. If you add, say what comes out.

Questions students ask about mapping

How many steps should the map have?
Enough that somebody who has never worked on your unit could carry the process out, which usually lands between ten and twenty for an ordinary clinical sequence. If your map has five steps, you have described a summary rather than a process, and the missing detail is almost certainly where the problem lives. If it has fifty, your boundaries are too wide and you should cut the sequence at a natural handoff. The test is not the count but whether every step names an actor and an action.
Can I map a process from a job I left?
Yes, provided you say so and treat the map as reconstructed rather than observed. Write one sentence stating when you worked there and that the sequence is drawn from experience, then be careful about claiming timings you cannot support. A recent process you can still watch is better material because the observation step is where most of the value in this week comes from. If your current role sits outside direct care, consider mapping a process you take part in from the other side, since a partial view honestly labeled beats a complete view invented.
Do I need special software for the diagram?
No. Boxes for actions, arrows for information moving, a lane per role if the assignment wants that, and a mark at the point you argue is the break, all of which can be drawn in a word processor or a slide program you already have.
