NR-505NP Week 4 is the reading week. This stage asks you to open the methods and results sections of a quantitative study and report what you found there: who was sampled and how, what instrument measured the outcome, what test was run, how large the difference was, and how much uncertainty sits around it. What NR-505NP Week 4 asks for
The territory is sampling, measurement and inference, and each of the three has a question attached. Sampling asks who ended up in the study and how far they sit from the patients your question is about, which means reading the inclusion and exclusion criteria, the recruitment route and the number who declined. Measurement asks what instrument produced the outcome numbers, whether it has been validated in this kind of population, and whether two studies claiming the same outcome measured it the same way. Inference asks what the analysis actually established, which is where p values, intervals and effect sizes do their separate jobs.
That separation is the graded core. A p value tells you how surprising the result would be if there were no real difference; it says nothing about how large the difference is or whether it matters to a patient. A confidence interval carries the size and the uncertainty together, which is why practitioner-focused appraisal leans on intervals. An effect size carries the magnitude in units a clinician can weigh. A paper reporting only significance has told you it found something without telling you whether the something is worth changing practice for, and saying that in your appraisal is worth real points.
Deliverable shape here is often a single study appraisal, sometimes with a standardized appraisal tool attached, sometimes as a structured critique with headed sections. Rebuild the sample from the criteria
Write one sentence describing who could enter and who was kept out, then a second sentence on how they were recruited. Consecutive patients from a clinic, volunteers responding to a notice and a random draw from a registry produce three different studies, and the recruitment route is where selection bias enters.

Follow the numbers from enrollment to analysis
Find how many entered, how many completed and how many were included in the analysis, and note where those three differ. Then look for whether the authors said anything about who left. Attrition that is unreported or lopsided between groups changes who the results describe, and naming it is one of the most reliably scored observations in the whole appraisal.

Interrogate the instrument, not the outcome name
Name the scale, questionnaire, laboratory value or chart definition that produced the outcome. Ask whether the authors reported any reliability or validity evidence for it in this population, and note when an outcome was self reported rather than observed, because that changes what the number means.

Match the analysis to the data
Check that the test suits the kind of variable being analyzed and the design that produced it. Continuous outcomes compared between two independent groups, proportions compared across categories and repeated measures on the same patients all call for different handling, and a mismatch is a legitimate appraisal finding even when you cannot re-run the analysis yourself.

Report size before significance
Write the magnitude first and the p value afterwards, if at all. A drop of a stated number of points on a named scale, with the interval around it, tells the reader something. A statement that the result was significant tells them only that the authors got past a threshold.

Convert the finding into patient terms
Finish by asking what the effect would mean for the patients in your question. Would this difference change a decision, a dose, a follow up interval? A statistically detectable change too small to alter management is a finding worth stating, and stating it is what the applicability row is waiting for.

Evidence craft for statistical reporting
Every number you quote needs its base. Write that a stated number of a stated total experienced the outcome across a stated follow up, rather than reporting a percentage alone. Then check whether the denominator is everyone enrolled or only everyone who finished, because those two denominators can tell opposite stories about the same trial.
Report the interval, not just the point. A difference with an interval that crosses no effect is a different sentence from one whose interval sits entirely on one side. Where the authors reported an interval, carry it into your prose; where they did not, say so, because an unreported interval is itself an appraisal finding.
Attribute the limitation to whoever noticed it. Keep the authors' acknowledged limitations visibly separate from the ones you identified yourself. Graders read the second set as your analysis and the first as your summary, and papers that blend them lose the credit for the thinking.
Statistical software is not a method. Naming the package tells the reader nothing about whether the analysis suited the data. Spend the sentence on what was compared, in whom, and with what test instead, and mention the tool only if your rubric asks for it.
Five mistakes that cost points in this week's territory
- Significance reported as size. A p value below a threshold says a difference is unlikely to be chance alone. It does not say the difference is large or clinically useful.
- The sample described as participants. Without criteria and recruitment route, the reader cannot judge whether anything here applies to your patients.
- Attrition passed over in silence. Who left a study, and whether they left evenly, changes what the surviving numbers describe.
- A generic limitations paragraph. Small sample size and single site, written without saying what those facts do to this conclusion, earns nothing.
- Outcome names treated as identical across studies. Two papers measuring adherence with different instruments are not measuring the same variable, and your synthesis later will inherit the error.

- Enrollment, completion and analyzed numbers are all reported
- The instrument behind each outcome is named, not just the outcome
- The main finding appears as a magnitude with its interval before any p value
- Every rate carries its numerator, denominator and follow up period
- Author acknowledged limitations and your own are visibly separated
- One paragraph states plainly what this study does not license you to claim
